When to seek help
A couple is generally advised to see a doctor if pregnancy has not happened after about a year of regular unprotected intercourse — or after six months if the woman is over 35. Both partners should be checked, because fertility depends on both.
The first test: semen analysis
A semen analysis looks at sperm count, movement (motility) and shape. It is simple, painless and gives a lot of information. Your doctor may add hormone tests or a scrotal scan depending on the result.
Common causes of low sperm count
- Varicocele — enlarged veins in the scrotum
- Infections, or past illnesses such as mumps
- Hormonal or thyroid problems
- Diabetes, obesity, smoking and heavy alcohol use
- Long-term stress and poor sleep
- Heat exposure (very hot baths, laptops on the lap, prolonged tight clothing)
- Some medicines, and rarely genetic causes
Lifestyle steps that support fertility
- Stop smoking and limit alcohol
- Keep a healthy weight and stay active
- Eat a balanced diet, sleep well and manage stress
- Avoid prolonged heat exposure to the groin

When to see a doctor promptly
Sudden testicular pain or swelling needs urgent medical care. Please also get a scrotal swelling that appears quickly, or one that is painful, examined without delay.
